Rheumatic Fever-Ayurvedic Concept

Introduction :
Term Rheumatic refers to pain in muscles, bones or joints. In Rheumatic Fever, fever will be accompanied with swelling, pain in the joints especially small joints.

One saying is there that “Rheumatic fever licks the joints, Bites the heart and kicks the Brain”.

Ayurvedic View :
In Ayurveda Rheumatic Fever is considered to be Amavatha. It is a dreadful disease which will make patient completely Bed ridden if not treated properly.

Clinical Features:
Clinical features of Amavatha are pain all over the body (Joints) loss of taste, thirst, lack of enthusiasm, heaviness, fever, indigestion and swelling of the body parts (joints).

Pathogenesis:

Ama (indigested food) associated with Vatha moves quickly to the different seats of Kapha in the Body, filling them and the dhamanies (Blood vessels) with waste materials. There the bad end product of digestion associated with vatha, pitha and Kapha assuming different colours, blocks the tissue parts and the passage with thick waxy materials. It produces weakness and heaviness of the heart which becomes the seat of the disease.

Treatment:

Since the origin of disease is in stomach first line of treatment should be to strengthened the digestive system, so as to remove amadosha (indigested food). For that purpose patient has to take drugs which are appetizers and laxatives, externally patient has to undergo Choornakizhi (Procedure in which patient is made to lie on a wooden table and strokes are given with medicated bolus by 2 trained masseurs for certain period).

Once the patient gets proper digestion and motion patient should undergo Elakizhi, pizhichil etc for the recuperation and complete recovery.

From my personal experience treatment procedure mentioned above has been very effective for Rheumatic fever patients and recurrence is very less.

Do's and Don't for Rheumatic Fever patients :-

  • Patient has to take complete bed rest
  • Should not take heavy foods which are hard to digest
  • Take only hot water
  • Avoid exercise
  • Avoid taking bath
  • Take gruel and light food

Blood Letting in Ayurveda(Raktha Moksha)

RAKTA MOKSHANA IN AYURVEDA

Rakta Mokshana is one of the pancha karmas. Rakta Mokshana is an unique contribution by Ayurveda. A number of diseases which are otherwise incurable can easily be cured through Rakta Mokshana (Blood letting). Susrutha has developed this as a specialized technique out of which Jaloukavacharna  has gained tremendous popularality even today, Rakta, the blood being the vehicle to carry & transport absorbed nutrients, Oxygen, metabolites etc.  From place to place. Correction of any abnormality in the blood by taking it out, Solves a number of problems.

Effect of Rakta Mokshana: -
Those who get Rakta Mokshana, regularly as a routine will never suffer from Twak Doshas. (Skin Problems), Grandhi ( Cysts & tumours), Sopha (inflammation), Rakta rogas ( Kusta, Nilika etc.,) Such is the miraculous effect of Rakta Mokshana.

How to Collect the Leeches:-

1. Select a river/ stream with fresh flowing water.

2.Annoint your leg by applying flesh all around the calf or wrap a freshly sloughtered piece of leather to your leg with its inner aspect exposed for leeches & stand a while by keeping your leg in water. Leeches will automatically come to suck the blood from the leather & get stuck – up to the skin.

3.Come out of water & pick up the leeches & preserve them in pure water.

Another method of collection of leeches is to send one or two buffalos preferably having wounds or boils on their skin & make them wait in water for about 15 minutes. Now take the buffalos out and you will find the leeches sucking blood here & there on buffalo’s skin.

Another easier method is, leeches also are caught in fishermen’s net. Thus leeches can be obtained from fishermen.

There are professional leech collectors, Where ever there are Zoology sections in degree colleges. They will supply many leeches on request. Any how, it is very difficult to differentiate poisonous leech on the basis of color etc as described above. The only criteria of selection should be pure, flowing water.

Preservation of Jaloukas (Leeches):-

After collection of required number of leeches keep the individual leech – In separate glass bottles. Provide them food in the form of algae, fungi, small leaves like Bhumyamalaki, cooked rice or fine pieces of mutton. Change the water once in two days. The water must be cool and container must be highly transparent for light rays. The container should not be kept in hot weather / sun. The container must be sufficiently big to be filled up with only three fourths of water allowing enough space for movements of leeches. The lid of container must have holes for exchange of air. Generally, one litre horlicks bottle is used for preserving leeches. The individual leech which is meant for application must be starved for seven days both before and after its application. A belly full leech fails to suck blood at the time of Jaloukava charana.

Stimulating Jaloukas:-

For the leeches to suck maximum amount of blood very quickly and swiftly without any problem, they must be stimulated and energized. For this, 1. Apply the paste of sarshapa + Rajani Kalka =Mustard and turmeric pastes to the leech. Take a rectangular tray and place turmeric powder & triphala churnam it. Drop the leeches in this water and you will find that an inactive leech becomes  highly active, and runs all around the tray which indicates its craving for food. Now , select the size of leech application on skin of the patient.

Procedure:

Give a small niche on the skin. Catch the leech at its neck with a toothless dissection forceps and apply its mouth to the bleeding point.  Incase , blood is not coming out of the skin, place a drop of honey or milk on the spot. After three  or four efforts, the leech will definitely stick -  up to the spot of Rakta Mokshana. Apply 10 – 15 such leeches depending up on the necessity and severity of disease. Knowledge of distinction between mouth and anus of a leech comes on experience or applying common sense. The leech when once starts sucking the blood, elevates its neck, and fixes its head to the supporting point of skin. You will find wave like movements indicating sucking of blood. Now, when you are sure that the leech is sucking the blood, cover its neck with a wet cotton and go- on dropping water continuously till its sucking is completed. Wait for 10 - 15 minutes for completion 0f its blood suction. Patient will not experience any discomfort as long as a leech sucks only his impure blood. After a few minutes, either the leech becomes fully satisfied with its food and leaves of the skin of the patient and drops  it self down , Or the patient may feel itching sensation which indicates impure blood form that spot is no more available for the leech.  In such a case, place a little turmeric powder ( Haridra churna) on the sucking point of the leech and immediately the leech will take away its mouth from that spot. Now, take out all such leeches after completing Rakta Mokshana and start producing emesis ( Vamana Karma) to the leeches.

Take out all leeches and dress the wound.
Keep the used leeches in an empty tray and go on placing turmeric powder on their mouths, You will find jets of black coloured blood being emitted by the  leeches.
Susrutha advises to squeeze out the blood with our fingers from anus to mouth. But the author has found that this method has resulted in the death of leech, probably because of injury to internal structures and strangulation. Hence, it is absolutely safe to produce emesis ( Vamana karma) by placing Haridra churna on their mouth.

Speciality of  Jaloukava Charana(leech craft):-

A leech has got an anticoagulant chemical called Hirudin. Thus a leech by sucking the blood sends out this Hirudin which prevents the patient’s blood from coagulation. And thus the leech can suck large amounts of impure blood without any problem. It is very difficult to clarify this in modern terms, the meanings of pure blood and impure blood.  More over, even to take toxaemic & Septicemic conditions to have impure blood (blood with bacteria etc) in circulation into consideration, it cannot be said whether a leech can separate or filter toxins & bacteria from the circulation. Any how, Ayurveda believes that a leech can definitely suck out only impure part of the blood of the patient. Accordingly, Ayurvedists achieved excellent results in several diseases through this jaloukava charana.

Medical Tourism in India
India is known in particular for Curative and Rejuvenative Ayurvedic Treatment, heart surgery, hip resurfacing and other areas of advanced medicine. The government and private hospital groups are committed to the goal of making India a world leader in the industry. The industry's main appeal is low-cost treatment. Most estimates claim treatment costs in India start at around a tenth of the price of comparable treatment in America or Britain.

Estimates of the value of medical tourism to India go as high as $2 billion a year by 2012. India has good, modern medical infrastructure that, in some places, can be comparable to medical infrastructure in the West. The Indian government is taking steps to address other infrastructure issues that can serve as a deterrant to the country's growth in medical tourism.

Ayurveda Healthcare
Kerala being the most health conscious State in the country, emphasis should be on “forecastive medicine” and “preventive therapies” for physiological and genetic diseases such as cancer, diabetes and mental disorders. The area of focus will be on early, sensitive and accurate diagnosis of the diseases (e.g. PCR based diagnostics; immuno diagnostics) and development of vaccines. The State will encourage the establishment of diagnostic service centers at District headquarters for infectious diseases such as tuberculosis and other pulmonary, parasitic and gastro intestinal infections and AIDS. Sree Chitra Thirunal Institute of Medical Sciences and Technology is already involved in ELISA and DNA based diagnostics for cardio vascular infections and genetic diseases. Rajiv Gandhi Centre for Biotechnology is active in PCR based diagnostics for entero, rota, hepatitis A and E viruses. Pharmacogenomics work at RGCB on software development for detection and identification of metabolic errors (e.g. single nucleotide polymorphisms) will complement this effort. SCTI and RGCB along with Regional  Cancer Centre will jointly spearhead the initiative with SCTI as the nodal institution.

Tissue Engineering has immense potential to be a niche area in medical biotechnology. Tissue Engineering is expected to revolutionise the technologies leading to organ replacement and the substitution of damaged tissues, e.g.,  in vitro regeneration of tissue including nerve, liver, bone and heart valves. The effort will involve the participation of biotechnologists and material scientists. Groups working at SCTI in the areas of polymer science and bioceramics will be encouraged to initiate the major programmes in this area.

Ayurveda and traditional medicines: Kerala is the cradle of Ayurveda and the traditional system of medicine is an integral part of the health regime of most Keralites. The Ayurvedic manufacturing sector in the State comprises nearly 760 units with 75 units having GMP certification (report by Ayurvedic Medicine manufacture’s Association). Total export earnings of the Ayurvedic medicines in 2001-2002 was Rs.750 crores and is expected to increase by Rs.5000 crores by 2005.

Medical tourism is a thriving activity in the State with huge employment potential. A leading Ayurveda company, for instance, has 322 formulations, 19 Ayur-clinics in and outside Kerala apart from a multi crore health resort in Bangalore and 60 franchise clinics. The Company markets nearly 300 classic formulations out of which 22 are patented drugs. A 40 acre herbal plantation near Bangalore as well as new ones in Orissa and U.P. cater to the company’s requirements. The above illustrative example is indicative of the tremendous potential for Ayurvedic medicine and medical tourism. The State Policy will encourage the synthesis of the traditional and modern in upgrading the quality and the content of the Ayurvedic medicines and services.

Dream Seasons - Kerala Tourism
The Tourism department of kerala has started promoting a new campaign called "Dream Season". This is mainly to attract tourism during offseason, i should say that this is the best time for medical tourism in kerala. Anyway the people who are coming to kerala for different kinds of treatments are charged very less when compared to other western countries, and if it is dream season(off season) in kerala then the hotels and resorts are also charged less compared to the peak season. In kerala the tourism offseason is from March to September.

This is a big opportunity for persons looking for medical care in kerala.

Kerala Tourism has come up with a `Dream Season' scheme to sell the traditionally lean April-September season.

The discerning tourist can enjoy Kerala at its flowery best during summer, and at its green best during the monsoon. Kerala Tourism is already on cloud nine, with a record arrival of 4.28 lakh foreign tourists during 2006-07. Tourism Director Sanjay M. Kaul says this is a record 23 per cent increase over the previous year. There was a five per cent increase in arrival of domestic tourists (63 lakh people). The World Travel and Tourism Council (WTTC) had during last year predicted an annual increase of 11 per cent in tourist arrivals to Kerala during the decade.

Aimed at promoting `Dream Season,' Kerala Tourism launched road shows in Mumbai, Ahmedabad and New Delhi. This has been complemented by advertisement blitzkriegs in print, Internet and radio. The monsoon is the best time for Ayurveda treatment.

Kerala is already a hot destination for tourists from the Gulf and northern parts of India during the rain, thanks to the summer heat in those places. The present aim is to attract tourists from outside the country as well. Tourism Secretary V. Venu says Kerala has been positioned as a reasonably-priced destination during the traditional lean months. "Hotels slash rates during the monsoon, to fit the purse of budget travellers. Now on, the Dream Season will be projected each year. Our aim is to make Kerala a 365-day destination."

The official website of the department - www.keralatourism.org has been spruced up to help tourists choose their destination, accommodation options and mode of travel in the State, during April-September. Over 150 organisations have been permitted to advertise their properties and activities on the website. The site speaks of how one can enjoy a milieu of choices, for Rs 4,444.
